How Somatic Therapy Relieves Chronic Pain & Illness

“Did I just think myself out of pain?” a client in her sixties asked me toward the end of a recent session.

She had come to me carrying years of chronic illness symptoms and persistent lower abdominal pain, layered on top of trauma from a life-threatening event decades earlier. For as long as she could remember, her body had been bracing against both her past and her present.

During our session, we didn’t use willpower to “fix” her condition, and she didn’t magically think her pain away. What happened was much simpler: her nervous system had finally been given permission to stand down. As her body stopped guarding against the discomfort, the physical pain that felt so permanent simply began to ease.

If you live with chronic illness, pain, or an autoimmune condition, you already know something most people never have to learn: your body doesn’t always feel like a safe place to be.

You wake up scanning for symptoms before your eyes are even fully open. You brace before you bend down, before you eat, or before committing to plans. Over time, that constant physical bracing becomes just as exhausting as the illness itself.

Chronic pain lives in the nervous system, not just the tissue

Fight, Flight, or Freeze

When pain or illness becomes chronic, something shifts beyond the original diagnosis. The nervous system is designed to sound the alarm during danger and turn off once safety returns. But it can sometimes get stuck in the “on” position. It keeps sending out those threat signals, keeping you in a low-grade state of fight, flight, or freeze.

A nervous system trapped in survival mode doesn’t just affect your mood. It directly impacts your digestion, sleep, inflammation levels, and how intensely you register pain. Clients often describe it as feeling like their body is constantly waiting for the other shoe to drop.

This is where somatic therapy comes in. We’re not just talking about the pain. We’re working directly with the nervous system that keeps pressing that alarm bell.

Somatic therapy isn’t “positive thinking” – it’s Physiological Safety

Calm the Stress Response

Somatic work for chronic conditions isn’t about pushing through discomfort or ignoring your diagnosis. It’s much slower and far gentler than that.

Here are some examples of what we might focus on in a session.

  • Uncoupling sensation from fear: Learning to notice a physical feeling without your nervous system immediately labeling it as an emergency. This helps your body learn the difference between a minor flare-up and a full-scale crisis.

  • Pendulation (moving between ease and discomfort): Gently shifting your attention back and forth between areas of tightness and areas of relative calm, showing your body that it is still capable of returning to a state of ease.

  • Small, manageable steps (titration): Processing subtle physical sensations bit by bit so your system never feels flooded or overwhelmed.

  • Releasing physical “guarding”: Using gentle movement and somatic breathwork to unwind the muscular tension that builds up from months or years of protecting injured or painful areas.

None of this erases a medical diagnosis, nor is it a substitute for medical care. But it calms the intense stress response layered on top of your illness. It is working to soothe the exhausted, high-alert state that can make a flare-up worse.

Moving from constant bracing to true recovery

Reset Your Nervous System

Remember my client who wondered if she had “thought” her way out of pain? Her pain didn’t lessen because her illness disappeared. Her illness was still there, but it eased because her nervous system finally stopped treating every physical sensation like a five-alarm fire. Once her body stopped dreading the pain, she had room to digest, sleep, think clearly, and simply exist without constant exhaustion.

When your nervous system learns that it doesn’t have to stay on guard 24/7, flare-ups tend to become less catastrophic, and recovering from them happens much faster.

If you’ve spent years being told to “just manage your symptoms,” or if you’ve started to accept exhaustion and worry as just your new normal, please know there is another way forward. Your nervous system can learn a new pattern. It takes time and gentle patience, but your body can become a place of safety again.
